Very rare case of type A acute aortic dissection

We describe a case of 46-year old man with very rare appearance of acute ascending aortic dissection. Computed tomography an­giography (CT angio) revealed aortic dissection limited to a short proximal ascending aortic segment with ‘apparently’ nor­mal aortic wall in the distal ascending aorta as well as proximal aortic arch, multiform flap of the aortic media within the distal arch and completely normal farther segments of the thoracic and abdominal aorta. Intraoperatively, a circular complete rupture of the aortic media two-three centimeters above aortic valve was noted. Apparently normal aortic wall of the distal ascending aorta and proximal aortic arch in CT scans was found to be an adventitia with few external layers of the dissected media. Intima and media of the distal ascending aorta was intussuscepted into aortic arch. Vascular prosthesis was implanted into ascending aorta and aortic arch in deep hypothermic circulatory arrest. Postoperative course was uneventful.
